A special commission found the accused perpetrators not guilty and they were released; only one person was convicted of involvement in the bombing, after pleading guilty in 2003 to manslaughter. The Governor General-in-Council in 2006 appointed former Supreme Court justice John Major to conduct a commission of inquiry and his report was completed and released on 17 June 2010. It was found that a "cascading series of errors" by the Government of Canada, the Royal Canadian Mounted Police, and the Canadian Security Intelligence Service had allowed the terrorist attack to take place
